Our post-hospital recovery program gives your family the breathing room you need. Your parent gets 24/7 professional oversight from caregivers trained in recovery support. Hospice Care Coordination
We are not the hospice nurse.
We are the ones who make sure your loved one is comfortable between visits.
When your family enters the hospice chapter, the medical team provides clinical care on their schedule. But your loved one needs support around the clock. They need someone to adjust the pillow, manage medications between nurse visits, keep them clean and comfortable, and simply be there. That’s where we come in.
We coordinate directly with your hospice provider and follow their care plan and comfort directives to the letter. Our job is not to replace the hospice team. It’s to be the steady, compassionate presence in between, so your loved one is never alone and you can focus on being family during the time that matters most.
Hospice Team Coordination
We work hand in hand with your hospice provider, attending team meetings, following their care plan, and ensuring every directive is carried out with precision and compassion.
